Would you like to become a patient with us? You can! On this page, you will find the registration requirements and the registration form. During our opening hours (Monday to Friday, 8:00 AM to 4:00 PM), you can contact our practice assistant by phone for any questions regarding registration.

Who can register with us?

You are eligible to register with us if:

  • You live in one of the following postal code areas: 1076, 1077, 1081, or 1082, or
  • You live in Uilenstede, or
  • You are a student at the Vrije Universiteit or enrolled in a related higher education program and reside in Amsterdam.

How to register?

There is a difference in registration procedure depending on whether you already have a GP in the Netherlands or not.

  1. First GP in the Netherlands

To register, simply fill out the form on our website, located at the bottom of this page. Please note that a Dutch or foreign health insurance is required for registration.

  1. You currently have another GP in the Netherlands

If you already have a GP in the Netherlands, you must inform them that you are switching to a new practice before registering with us. To register, complete the form at the bottom of this page. Once your registration is processed, we will electronically request your medical file from your previous GP. You can schedule an introductory appointment with your new GP only after we have received your medical file. If you require an earlier consultation due to an emergency, you can book an appointment. Please ensure that your home address matches the address registered with your health insurance.
